Skills are sword/tactics/bushido/parry/anatomy/healing (although not all skills are at 120 at the moment).

What is the best pvp weapon for this char?
I believe it will be 2H weapon (so he could get 1 reforged + 4 other mods at max intensity).
I could imbue Mana Drain, Hit Fatigue, Reactive Paralyze, etc. Or some mod at 140% intensity or may be 50 Mana Drain + 40 Hit Fatigue.

Any ideas? Notes about tactic are welcome as well.

You'll probably die before you can hit anyone if you use a 2-handed weapon, unless you're planning on making it Balanced.

Missed that he said PvP. :(

Yeah very rarely recall seeing a two-hander in PvP. Not since the days when some people would use the Axe of the Heavens as a finisher.

Potions are pretty much essential in PvP unless you're running a specialised mage or 4/6 Paladin which uses both hand slots, simply because of the amount of burst damage that can be put down on you as well as the need to refresh stamina and counter poisons quickly.

They have an impact but it is nerfed compared to PvM.

Most people do not appear to bother with them.

    So how about
    50 Hit Mana Drain
    40 Hit Fatigue
    50 DI
    50 Hit Fireball
    Balanced

No SSI? Not sure if they've been 'fixed' recently but when I tested them Hit Mana Drain and Hit Fatigue had negligible effects in PvP.
