One of my in-game friends (thanks Rose!) pointed out the following thread on the forums that describes the issue as essentially the critical results are just providing shards and not an relics of the next higher tier.

Celestrata has confirmed this issue and they’re working to fix it but for now, hold off on combining relics!  But if you’ve already been bit with this, you should try to provide as much information as possible regarding the tiers and such.
